Pharmacist Prescriber at Cambridge Medical Centre

At Cambridge Medical Centre we have a Pharmacist Prescriber who supports the general practice team.  Their role is to manage and monitor medications and provide specialist advice and input into your patient care plan. 

You can either ask to speak to our Pharmacist Prescriber directly or you can talk to a doctor or nurse about whether meeting with the Pharmacist Prescriber would be helpful for you.  

Pharmacist Prescriber Services

At Cambridge Medical our Pharmacist Prescriber works with you and your Doctor on:

  • Getting the best from medicines

  • Assistance if you have had any medicine changes after recently being in hospital

  • Blood Pressure Management

  • Diabetes Management

  • Cholesterol Management

  • Asthma and COPD Management

  • Gout

  • Providing information on your medicine or helping you access more information about your medicines

  • Links with your community pharmacy to make it as easy as possible for you to take your medicines and understand what they are

Ask to see the Pharmacist Prescriber....

  • If you are having problems with your medications such as side effects, hard to use medicines, or you are taking a large number of medicines

  • If you have been to hospital recently

  • If you need help managing diabetes, high blood pressure or Asthma/COPD

  • If your medicines are causing you problems

  • If you would like some education about your medicines and to check if you are taking them correctly

  • If you want to know if Blister Packs are right for you or someone you care for

  • If you would like to have your inhaler technique checked

What should I bring with me to see the Pharmacist Prescriber?

Please bring all the medicines you take which includes:

  • Medicines prescribed by your doctor

  • Medicines you buy from the pharmacy or supermarket

  • Vitamins, supplements or herbal products

  • Rongoa or traditional medicines

You are also welcome to bring a support person if you think this will be helpful for you,

Transition of Care

Have you or someone you care for recently been discharged from hospital with changes to medications? 

Our Clinical Pharmacist can go through your medications with you, explain any changes and reconcile any discrepancies, in partnership with your GP, which may have occurred. 

Our Pharmacist Prescriber is available for consultations to review your current medications.

The aim of these reviews are to discuss your healthcare goals, optimise your medications, increase your knowledge and understanding, and to find ways to make taking your medications (and remembering to take them) easier if required.

Our Pharmacist Prescriber can also complete your annual review for long term medications to ensure your repeat prescriptions continue.
